    Pre-participation Physical Evaluation Form
This completed form must be kept on file by the school. This form is valid for 365 calendar days from the date of the evaluation as written on page 2.


 
Consent and Release from Liability Certificate 
This completed form must be kept on file by the school.

ATTENTION PARENTS: Requirements for your son or daughter to participate in Athletics here at Father Lopez:

1.)  The physical form and parent consent form must be signed and dated by both a parent and the student-athlete who wishes to participate.

2.)    Please list the sport(s) that your son or daughter will be participating in for the year in the designated areas on the forms.
