The following (2) forms must be submitted to the Athletic Office before any student will be permitted to participate in interscholastic sports.
Pre-participation Physical Evaluation Form
This completed form must be kept on file by the school. This form is valid for 365 calendar days from the date of the evaluation as written on page 2.
Consent and Release from Liability Certificate
This completed form must be kept on file by the school.
